What follows is often weeks of low mood, flat affect, poor concentration and intense cravings. Depression and suicidal thinking in this window are a genuine clinical concern and are one of the main reasons stimulant withdrawal is better done in a supervised setting than alone at home.

Benzodiazepines - prescribed or otherwise

The highest medical risk of the group. Abrupt cessation after sustained use can cause seizures, and withdrawal can also involve severe anxiety, perceptual disturbance and confusion. Benzodiazepines are normally reduced gradually over weeks or months under medical supervision rather than stopped. Cannabis

Frequently dismissed, but withdrawal is real in heavy daily users: irritability, anxiety, disturbed sleep and vivid dreams, reduced appetite and low mood, generally over one to two weeks. Not medically dangerous, but uncomfortable enough to drive a return to use.

Mandrax

Withdrawal from methaqualone-containing substances can include anxiety, agitation, insomnia and, in heavy users, seizure risk. Because Mandrax is frequently smoked with cannabis and often used alongside other substances, assessment should cover everything being used.

Physical Versus Psychological Withdrawal

The two are often confused, and it matters clinically. Physical withdrawal is what the body does when a substance is removed - it has a defined timeline and, for some drugs, real medical risk. Psychological withdrawal is the craving, low mood and preoccupation that follows, and it typically outlasts the physical stage by weeks or months. Most relapse happens in the psychological phase, well after detox is finished.

Post-Acute Symptoms

After acute withdrawal settles, many people experience a longer period of disturbed sleep, mood swings, poor concentration, low energy and episodic strong cravings. This can persist for months, particularly after long-term stimulant or opioid use. It is a recognised part of recovery, it improves with time, and it is a common trigger for relapse in people who expected to feel normal after a week.

Overdose Risk After Detox

This deserves stating plainly. Tolerance drops quickly during withdrawal. Someone who returns to their previous dose after a week clean is at substantially higher overdose risk than before - this is a leading cause of death following opioid detox. Anyone leaving detox early, or discharged without a follow-on plan, should be aware of this, and the people around them should be too.
